A tragic head-on collision in Prescott on Saturday afternoon claimed the lives of a father and two of his children, while four others were hospitalized, according to the Yavapai County Sheriff’s Office.
Authorities reported that the crash occurred around 3:01 p.m. on Williamson Valley Road near Levie Lane, an area located west of Prescott Regional Airport and east of Granite Mountain. When deputies arrived, they found Jesse Forman, a Chino Valley resident, and one of his children deceased at the scene. A second child succumbed to injuries while being transported to the hospital.
The crash involved Forman’s Toyota 4-Runner, which was carrying his three children and their mother, and a Jeep SUV occupied by a woman in her 60s and her adult son. Investigators say Forman’s vehicle veered across the center line, leading to the devastating head-on impact.
Emergency responders airlifted three adults to hospitals for treatment, including the woman and her son, who were taken to a Flagstaff hospital with serious injuries. A third child from Forman’s vehicle remains in critical condition at a hospital in Phoenix. Authorities have not disclosed the severity of the mother’s injuries.
Sheriff David Rhodes extended his condolences to the victims’ families and acknowledged the first responders for their swift efforts in aiding those involved. He also reaffirmed the department’s commitment to increased traffic enforcement in the area, citing ongoing concerns about reckless driving on Williamson Valley Road.
The investigation into the crash remains ongoing.
