The Cuyahoga County Medical Examiner has identified the deceased as Patrick Kerr, a forty-five-year-old Cleveland resident who sustained fatal injuries after law enforcement personnel discharged gunfire during a multi-hour tactical operation. The incident unfolded along Tiedeman Road and the I-480 freeway corridor on Tuesday.

Cleveland authorities initiated pursuit actions shortly before the clock struck eleven in the morning, chasing a black F-150 truck that subsequently collided with other automobiles in the vicinity. This pursuit triggered a law enforcement negotiation process that extended into the subsequent hours.

The individual involved in this extended interaction was identified as Patrick Kerr. According to official reports, Mr. Kerr presented a significant danger to himself and others, specifically threatening self-harm and having previously exhibited concerning behaviors related to suicidal ideation involving law enforcement personnel. During the incident, Mr. Kerr exited his vehicle while visibly armed.

Police forces ultimately responded with tactical gunfire, resulting in Mr. Kerr’s immediate death. Medical personnel at MetroHealth Medical Center confirmed his passing. Separately, one of the other drivers involved in the accident sustained severe injuries leading to death at the scene, identified as Ronald James Liszka, a sixty-two-year-old resident of Brooklyn. Emergency medical services transported three other individuals to the hospital for treatment.

An independent investigation has been commissioned by the Ohio Bureau of Criminal Investigation to examine the officer-involved shooting incident.
