David Antonio, 31, of Springfield, died Monday evening following a two-vehicle collision at the intersection of State Route 4 and State Route 334 in Clark County, Ohio State Highway Patrol officials reported. The crash occurred around 6:34 p.m. when Antonio’s Saturn Ion failed to yield at a stop sign and collided with a southbound Honda Accord operated by 18-year-old Gavin Dyer of Urbana.

The impact caused both vehicles to leave the east side of SR-4. Antonio was pronounced dead at the scene, while Dyer and his passenger, 18-year-old Adisyn Welty of Urbana, were transported to Springfield Regional Medical Center with minor injuries.

Authorities temporarily closed the intersection for the investigation, reopening it shortly after 8:40 p.m. The Ohio State Highway Patrol continues to investigate the circumstances surrounding the crash.
