The collision occurred in the 2300 block of West Jefferson Boulevard near Swinney Park, prompting authorities to temporarily close part of the busy roadway west of downtown while emergency crews responded.
Investigators with the Fort Wayne Police Department said Forker was driving when her vehicle suddenly crossed into the oncoming lane for unknown reasons and struck another car head-on.
