Fatal Wasatch County, Utah car crash death of Don C. Herbert and Mitchell L. Farnsworth – Obituary

Authorities have identified the two men who were killed in a head-on collision on State Route 40 in Wasatch County on March 14. The victims were 39-year-old Don C. Herbert and 34-year-old Mitchell L. Farnsworth.

According to the Utah Highway Patrol, the crash occurred around 2:30 p.m. when an eastbound box truck veered onto the dirt shoulder before reentering the roadway and crossing the centerline. The truck collided with a westbound Chevy Silverado at approximately milepost 51. The impact caused the cab of the box truck to detach from its frame and overturn.

Both occupants of the Silverado sustained fatal injuries and were pronounced dead at the scene. The driver of the box truck was transported to a hospital with serious injuries, though authorities have not released an update on his condition.

Investigators continue to examine the circumstances surrounding the crash, and additional details have not yet been released.
