Fatal Waukegan pedestrian accident death of Juan Mentada – Obituary
The coroner has confirmed that Juan Mentada, a 43-year-old Waukegan resident, died after being struck by an SUV while attempting to cross the street early Saturday morning. An autopsy conducted on Tuesday revealed that his death was due to blunt force injuries sustained in the collision.
Emergency responders from the Waukegan Fire and Police Departments arrived at the intersection of Grand Avenue and Berwick Boulevard around 12:30 a.m. on Saturday to find Mentada injured. According to Waukegan Deputy Police Chief Scott Chastain, Mentada had narrowly avoided being hit by several vehicles before he was struck by a 2018 Nissan Rogue.
The Nissan, traveling in the eastbound lanes, stopped immediately after the accident. Mentada was subsequently transported to Vista Medical Center East, where he was pronounced dead.
Initially, police reported Mentada as a Park City resident; however, the coroner confirmed he lived in Waukegan. The driver of the SUV, a 20-year-old man from Park City, remained at the scene and cooperated fully with the investigation. He appeared not to be impaired but provided toxicology samples for further analysis.
The Waukegan Police Department Major Crash Unit is continuing to investigate the circumstances surrounding the incident.
